OPERATOR LICENSE
It is your responsibility to complete the entire form for each application you submit! The City Hall office is here to assist you, not complete the forms for you! The following forms need to be completed and filed with the City under the described conditions. Please read the description carefully to determine what applications need to be filled by your entity. Applications are due by Tuesday, May 12, 2010. PLEASE NOTE THAT ALL FORMS LISTED UNDER EACH TYPE NEED TO BE FILED WHETHER IT IS A NEW OR A RENEWAL APPLICATION! NEW APPLICANTS NEW LICENSE (Individual or Partnership) You Need to File: form AT-106* Original Beverage License; and form AT-103 Auxiliary Questionnaire, one for each individual. NEW LICENSE (Corporation/LLC) Corporations must apply for licenses under the name for which they are registered with the Secretary of State’s Office. A corporation must have at least two officers; namely, a president and a secretary who are not the same person. The president and vice president of the corporation also CANNOT be the same person. You Need to File: form AT-106* Original Beverage License; form AT-103 Auxiliary Questionnaire, one for each officer and the Agent; form AT-104 Appointment of Agent. * BE SURE TO PROVIDE YOUR SELLER’S PERMIT NUMBER AND FEDERAL EMPLOYER ID NUMBER IN THE AREA PROVIDED ON THE UPPER RIGHT CORNER OF THE FORM. RENEWAL APPLICANTS RENEWAL LICENSE (Having NO CHANGES in Individual name, partners, officers etc.) You Need to File: form AT-115* Renewal Beverage License and form AT-103 Auxiliary Questionnaire, one for each individual. RENEWAL LICENSE (Having NO CHANGES in the Corporation or Partnership) You Need to File: form AT-115* Renewal Beverage License; form AT-103 Auxiliary Questionnaire, one for each officer and the Agent; and form AT-104 Appointment of Agent. * BE SURE TO PROVIDE YOUR SELLER’S PERMIT NUMBER AND FEDERAL EMPLOYER ID NUMBER IN THE AREA PROVIDED ON THE UPPER RIGHT CORNER OF THE FORM. Class A or B Beer Licenses & Class C Wine $100.00 Class A or B Liquor Licenses $500.00 Publication Fee** $ 25.00
** Publication fee is $25.00 dollars if submitted by Tuesday, May 12, 2010. Failure to submit by this date will cause an increase in the publication fee and may jeopardize your ability to operate or to renew your license. Please remind anyone holding an operator’s license that renewal applications must be submitted to the City Clerk’s office for Common Council approval. For approval on the June 1, 2010 meeting the application must be in our office by May 28, 2010. For approval on the June 15, 2010 meeting the application must be in our office by June 11, 2010. If applications are not received by these dates, the applications will be considered “NEW” and require a personal appearance before the Regulatory Committee and an additional $7 fee for a background check. Bartender’s License (1 yr/2 yr RENEWAL) $25.00/$35.00 (1 yr/2 yr NEW) $32.00/$42.00 Please post this operator reminder poster in an accessible location for all your bartenders as a reminder to renew their operator license. Operator's licenses approved on June 1, 2010 meeting will be sent to the establishment listed by applicant unless otherwise specified. The following links are lists of Operator's Licenses that will Expire June 30, 2010. Applicants with just a cigarette license must pay the fee when application is submitted. For applicants submitting liquor license applications, payment at time of submittal is not required only publication fee is due. Cigarette License Fee $100.00 March 1, 2010 Notification Letter of Renewal mailed out to establishments. May 12, 2010 Beverage applications are due. The publication fee of $25.00 is due at this time. May 28, 2010 Renewal operator license applications due for the June 1, 2010 meeting.
Last day to submit "LATE" beverage applications for June 15, 2010 meeting. An increased publication fee will be assessed. June 1, 2010 COMMON COUNCIL MEETING June 4, 2010 Approved applications can be picked up at City Hall after 1:00 p.m. The remaining balance is due at this time. June 11, 2010
June 15, 2010 COMMON COUNCIL MEETING June 18, 2010 Approved applications can be picked up at City Hall after 1:00 p.m. The remaining balance is due at this time. June 30, 2010 ALL LICENSES EXPIRE AT MIDNIGHT Applications must be picked up at City Hall by 5:00 p.m. to operate at 12:00 a.m. July 1, 2010. Submit completed forms to the City Hall Office, 317 South Main Street, Jefferson, WI 53549.
